PAYMENT INFORMATION - Traveling Payment of $495 is due for Traveling Teams before teams are posted. Credit Cards and Checks are not deposited until after tryouts. We will contact you during or after tryouts for payment if your child is selected to play on a Traveling Team. Some parents may wish to leave a post dated check or credit card number if they are going to be out of town after the last day of tryouts.
PAYMENT INFORMATION - Non-Traveling Payment of the $249 is required for Non-Traveling Teams at the time of registration. (NOTE: Girls trying out for the Travel AND Non-Travel Teams will have their $249 fee applied to their $495 Traveling Team fee if they make a Traveling Team.) Once your Registration Form has been received and processed, you will receive an Email or Phone Call with instructions to log into our secured online system to process your CJV Payment. Or you can mail your payment to Carolina Juniors PO Box 470881, Charlotte NC 28247. Parent's must have an Email address on file to access the online program.
CONSENT FORM All participants must have a Signed Release and Waiver Of Liability and Indemnity Agreement and Consent Form on file with Sports Connection before they will be allowed to participate in any Leagues or Other Activities associated with the Sports Connection, LLC. Participants under 18 years of age must have a parent or guardian sign the Sports Connection, LLC Release and Waiver Of Liability and Indemnity Agreement. I, (self/parent/guardian), desire for my (self/son/daughter) to participate in one of Sports Connection, LLC's or Charlotte Sports Center, LLC's leagues or other activities and have read and agree to the terms and conditions listed below regarding the Release and Waiver of Liability and Indemnity Agreement. I understand that playing in a sports league or other activity involves physical contact between players and I assume the risks associated with (my/his/her) participation. (I/My child) (have/has) no known medical conditions which prohibit participation. I agree to hold harmless and release Sports Connection, LLC, Charlotte Sports Center, LLC and their agents and sponsors from legal liability resulting from any injuries sustained during participation in these activities. I understand Sports Connection, LLC and Charlotte Sports Center, LLC cannot be responsible for loss or damage to (my/my child's) personal possessions.
